Patients - New Visits

Thank you for choosing the Northwest Vein & Aesthetic Center. We are committed to the success of your medical treatment and care.

Please fill out the forms indicated carefully and completely, and bring them with you to your appointment, along with your current insurance card(s).

You may also want to bring a pair of loose fitting shorts to wear during your exam (gowns are provided if you prefer).

Our office is located at St. Anthony Medical Building. Before your sceduled appointment we will send an information packet. Please allow 2 hours for you initial visit, since we have combined 3 visits in one. Often a diagnostic vascular ultrasound is needed to determine the treatment needed. At the end of your visit we will be able to formulate a treatment plan, that best suits your individual needs.
